The aim of the present study was to investigate the association between work-related moral stress, moral climate and moral sensitivity in mental health nursing. By means of the three scales Hospital Ethical Climate Survey, Moral Sensitivity Questionnaire and Work-Related Moral Stress, 49 participants’ experiences were assessed. The results of linear regression analysis indicated that moral stress was determined to a degree by the work place’s moral climate as well as by two aspects of the mental health staff’s moral sensitivity. The (...) nurses’ experience of ‘moral burden’ or ‘moral support’ increased or decreased their experience of moral stress. Their work-related moral stress was determined by the job-associated moral climate and two aspects of moral sensitivity. Our findings showed an association between three concepts: moral sensitivity, moral climate and moral stress. Despite being a small study, the findings seem relevant for future research leading to theory development and conceptual clarity. We suggest that more attention be given to methodological issues and developing designs that allow for comparative research in other disciplines, as well as in-depth knowledge of moral agency. Research on ethical dilemmas in health care has become increasingly salient during the last two decades resulting in confusion about the concept of moral distress. The aim of the present paper is to provide an overview and a comparative analysis of the theoretical understandings of moral distress and related concepts. The focus is on five concepts: moral distress, moral stress, stress of conscience, moral sensitivity and ethical climate. It is suggested that moral distress connects mainly to a psychological perspective; stress (...) of conscience more to a theological–philosophical standpoint; and moral stress mostly to a physiological perspective. Further analysis indicates that these thoughts can be linked to the concepts of moral sensitivity and ethical climate through a relationship to moral agency. Moral agency comprises a moral awareness of moral problems and moral responsibility for others. It is suggested that moral distress may serve as a positive catalyst in exercising moral agency. An interdisciplinary approach in research and practice broadens our understanding of moral distress and its impact on health care personnel and patient care. (shrink)

The interconnection between moral distress, moral sensitivity, and moral resilience was explored by constructing two hypothetical scenarios based on a recent Swedish newspaper report. In the first scenario, a 77-year-old man, rational and awake, was coded as “do not resuscitate” (DNR) against his daughter’s wishes. The patient died in the presence of nurses who were not permitted to resuscitate him. The second scenario concerned a 41-year-old man, who had been in a coma for three weeks. He was also coded as (...) “do not resuscitate” and, when he stopped breathing, was resuscitated by his father. The nurses persuaded the physician on call to resume life support treatment and the patient recovered. These scenarios were analyzed using Viktor Frankl’s existential philosophy, resulting in a conceivable theoretical connection between moral distress, moral sensitivity, and moral resilience. To substantiate our conclusion, we encourage further empirical research. (shrink)

Hegel's Science of Logic has received less attention than his Phenomenology of Spirit, but Hegel himself took it to be his highest philosophical achievement and the backbone of his system. The present book focuses on this most difficult of Hegel’s published works. The care for older and disabled people has been described as a core area of the Nordic model. The Nordic countries’ welfare model has also been described as women friendly, as women are not forced to make harder choices than men between work and family. The Swedish eldercare system has, during the last several decades, undergone significant changes. Previously, eldercare could be described as universal, meaning a publicly provided, comprehensive, high-quality service available to all citizens according to need and not (...) based on the ability to pay. In later years transformation of eldercare has been influenced by neoliberal politics, which emphasize economic efficiency and cost reduction through competition. Eldercare has become a more diverse multidimensional system, and a private market for home-based eldercare has been created. The numbers of eldercare providers have increased considerably, and new ways of organizing eldercare have been established. In January 2009, the Act on System of Choice in the Public Sector was introduced. The Act was supposed to provide an opportunity for interested municipalities and county councils to expose their publicly provided services to market competition, and to enable users to choose their providers. This article aims to illustrate how neoliberal reasoning dominated the policy process leading to adoption of the Act on System of Choice in the Public Sector. With the use of a discursive policy analysis the authors specifically explore how neoliberal logic dominated, and also how choice and equality were understood and interpreted in the policy process. They conclude that the neoliberal turn in eldercare claiming to centre on the individual choice of persons in need of care runs the risk of creating unequal care that decentres the eldercare worker and creates precarious work situations. What relevance does Foucault have, more than a decade after his death? Foucault was a sort of philosophical journalist - continually concerned with what is happening in the present. And it is here that we find one of the guiding threads of Foucault's ethics: we must be constantly vigilant in ensuring that the present does not become a mere repetition of the past. Philosophy must produce events that can act to disrupt this repetition. This is the task of judgment, confronted (...) by the question of power. We can recognize four points on which Foucault has continuing relevance. First, Foucault diagnosed the end of revolution, the consequences of which we are now living. Further, Foucault's work on truth-telling and norms and measures, the heart of the question of justice, is still significant. Foucault's work on medicine, and in particular ancient Greek medicine, will also play a critical role if we are to successfully navigate the myriad problems posed by a new form of medicine, genetic medicine - a medicine of predispositions rather than therapy, similar to the medicine practiced by the Greeks. Fourth and finally, Foucault has a continuing relevance for problems of decision-making, responsibility and care. We can think about responsibility, for Foucault, through the question of care - and thus will avoid reducing responsibility to a merely juridical notion. From 802 to 811, we encounter several diplomatic missions beween Byzantium and the Carolingians with the scope to secure or confirm peace, although in 798 such an agreement had been reached. The real target behind such negotiations was a recognition of the coronation and imperial title of Charlemagne since 800, denied by Byzantium for years. It was only in 810 that Nicephorus I yielded due to military/ political difficulties in Northern Italy with Pepin/pippin, son of Charlemagne, and against the Bulgarians. (...) A treaty defined spheres of interest and Byzantine representatives acclaimed Charlemagne as basileus in 812. The present article tries to clarify the different stages and elements of the whole process. When the power of the Carolingian state diminished in the ongoing 9th century, Byzantium step by step deviated from full recognition of the Western emperor, a process clearly visible during the joint venture against Arab Bari from 869 to 871. An appendix deals with the question, if Empress Eirene had offered a kind of junior-emperorship to Charlemagne in 798/799, as already Paul Speck proposed more than forty years ago, without finding much approval. The article reconstructs three key concepts of Hans Vaihinger: the idea of mental fictions as self-contradictory, provisory, conscious, and purposeful; the law of the devolution of ideas stating that an idea oscillates between dogma, hypothesis, or fiction; and the underlying assumption about human consciousness that the psyche constructs thoughts around perceptions like an oyster produces a pearl. In a second, constructive part, these concepts are applied in a discussion of John Searle’s social ontologically extended theory of speech acts. The article (...) introduces the Vaihingerian as-if to Searle’s account of declarations. The explanatory work in a model of social reality as Searle has proposed it rests on the ability to show a necessary connection between collective and individual intentionality facilitated through linguistic structure. The methodological individualism of the model requires that motivational assumptions about collective structures be realized in individual brains. In the first part of the book she discusses contemporary analyses of our use of 'I' in language and thought, and compares them to Kant's account of self-consciousness, especially the type of self-consciousness expressed in the proposition 'I think.' According to many contemporary philosophers, necessarily, any instance of our use of 'I' is backed by our consciousness of our own body. For Kant, (...) in contrast, 'I think' just expresses our consciousness of being engaged in bringing rational unity into the contents of our mental states. In the second part of the book, Longuenesse analyzes the details of Kant's view and argues that contemporary discussions in philosophy and psychology stand to benefit from Kant's insights into self-consciousness and the unity of consciousness. The third and final part of the book outlines similarities between Kant's view of the structure of mental life grounding our uses of 'I' in 'I think' and in the moral 'I ought to, ' on the one hand; and Freud's analysis of the organizations of mental processes he calls 'ego' and 'superego' on the other hand. Longuenesse argues that Freudian metapsychology offers a path to a naturalization of Kant's transcendental view of the mind. It offers a developmental account of the normative capacities that ground our uses of 'I, ' which Kant thought could not be accounted for without appealing to a world of pure intelligences, distinct from the empirical, natural world of physical entities. This paper investigates the link between the total bivariate risk premium and the sum of partial bivariate risk premia. Whereas in the case of small risks, the non interaction between risks is a sufficient condition to obtain the equality between the total risk premium and the sum of partial risk premia, the paper shows that this condition is not sufficient for large risks. The non interaction between risks occurs in two cases: if risks are independent or if individual's marginal utility (...) of one good is independent of the endowment in the other. Without restriction on the utility function, none of these two conditions is sufficient for large risks. If attention is restricted to preferences that exhibit constant absolute risk aversion, the non variability of the marginal utility of good one with respect to variations in endowment in the other remains a sufficient condition, while the independence between risks does not. : The question as to whether or not it was allowed to dye one’s hair, became heavily discussed in early Islam. So we find controversial traditions on this problem. Dyeing the hair however, was not only under discussion in religious circles, but also in topics taken by poets. In Arabic poetry, grey hair meant old age. As a consequence, it was the end of love and youthful life. Dyeing the hair therefore, was a means to cheat the ladies about the (...) real age of the poet. The poets did not have so much religious scruples, since for them the effectiveness of dyeing was the point. On this however, some poets had their doubts. The article gives an overview on how Arabic poets treated the pro and contra of dyeing the hair – some doing it in a self-ironical way. (shrink)

This paper presents the case of the post-crisis discursive defence of shadow banking in the Netherlands to argue, first, that there is a need to dust off older elite theories and adapt them to post-democratic conditions where there are no widely shared ‘political formulas’ to secure mass support for elite projects. Second, that temporality should be taken more seriously; it is when stories fail that elite storytelling can be observed in practice. As new ‘political formulas’ are minted and become established, (...) elites can again hope to withdraw from the political scene and leave policy-making to the self-evidence of output legitimacy and/or the perpetuum mobile of There-Is-No-Alternative. This suggests that elite theory should replace an epochal reading of post-democracy with a more conjunctural one. (shrink)
